When this selection is made, most of the data is already populated for the user from their stored profile. The user may choose the "severity" from the drop-down menu to alert the engineer as to the importance of the issue. The user has further selections at the bottom of this form to describe in detail to the technician exactly the issue, including attaching a screen-shot or other attachment to the ticket. Once the "create" button has been selected, the user will be taken to a confirmation screen advising them of the ticket ID assigned to the report.
